What is the Commonwealth Home Support Program (CHSP)?

The entry- level CHSP program helps older people in the community maintain independence with basic supports to suit individual needs and preferences.

The CHSP promotes wellness and build on strengths and abilities. Support can be short term (e.g. after leaving hospital) or long term.

The CHSP also provides planned respite for eligible people to allow carers to take a break.

What is a Home Care Package (HCP)?

A Home Care Package provides coordinated, personalised care at home. There are four levels of packages to support people with basic, low, intermediate or high care needs.

HCPs include personal care, support services at home and community, nursing, allied health and clinical services.

HCPs include case management, and your service provider will work with you to identify care and services to meet your particular needs.
