Annual fees

Organisation: $275 (GST inclusive).

Sole Trader: $99 (GST inclusive).

PLEASE NOTE: in recognition of the challenging circumstances during the coronavirus (COVID-19) pandemic, COTA Tasmania is waiving the annual provider fees until further notice.

Insurance

The Provider or their authorised representative understands that they must have the following insurance (you must tick each box to confirm):

Reporting

The Provider is required to report, on a quarterly basis:

• the number of initial assessments conducted (i.e. new participants) during the period;
• the number of Living Longer Living Stronger sessions delivered during the period;
• the number of participants that did five (5) or more sessions during the period; and
• the number of participants that have did less than five (5) sessions during the period.

In order to report on these details, the Provider will need to keep records of: initial assessments undertaken; sessions delivered; and session attendance by participants.

Reporting is facilitated through a simple online form, which is to be completed after each quarter. Providers will be contacted via email to remind them of the reporting requirements each quarter.

By submitting this Agreement, the Provider or their authorised representative confirms that they understand and will comply with the reporting requirements.

Accreditation

All Instructors delivering the Program must have:

• successfully completed the Living Longer Living Stronger Instructor Accreditation Course or the Strength for Life Instructor Accreditation Course; and
• hold current Living Longer Living Stronger accreditation or current Strength for Life accreditation.

By submitting this Agreement, the Provider or their authorised representative confirms that they understand and will comply with the requirement that only Living Longer Living Stronger or Strength for Life accredited instructors will deliver Living Longer Living Stronger sessions or initial assessments.

Qualifications

The Provider’s Instructors who deliver the Program must hold and maintain the appropriate qualification(s), registration(s) and licence(s) and provide verification of these on request.

Fitness professionals must have:

• Certificate III and/or IV in Fitness qualifications.

Exercise physiologists must have a:

• Qualification as an exercise physiologist that is recognised by Exercise & Sports Science Australia.

Physiotherapists must have a:

• Qualification as a physiotherapist that is recognised by the Australian Health Practitioner Regulation Agency and its National Board, the Physiotherapy Board of Australia.

By submitting this Agreement, the Provider or their authorised representative confirms that they understand and will comply with the qualification requirements for instructors.

Living Longer Living Stronger is an evidenced based strength and balance training program that aims to improve health outcomes, quality of life and fitness for older people. The program is designed to cater to a diverse range of people and fitness levels, and promote social inclusion and engagement.

Provider Responsibilities

Providers are required to have the organisational capacity to deliver a progressive strength and balance training program at a site (or online) where all participants, regardless of age, mobility, background or circumstance, feel valued, respected and safe.

The Provider is required to comply with industry standards and will:

• Maintain a safe and age friendly environment, and ensure that equipment is in good working order.
• Have Professional Indemnity and Public Liability insurance cover.
• Ensure that participant fees are within the required caps.
• Ensure that all personal and medical information of participants is kept confidential and stored securely.
• Report quarterly to COTA Tasmania on the number of: initial assessments conducted; Living Longer Living Stronger sessions delivered; participants that attended five or more sessions; and participants that attended less than five sessions.
• Ensure that all instructors delivering the program have the required qualifications and Living Longer Living Stronger or Strength for Life accreditation.
• Ensure that each participant undergoes an initial assessment conducted by a Living Longer Living Stronger or Strength for Life accredited instructor.
• Deliver the program in a group setting, with a maximum of 15 participants per instructor.
• Allocate an appropriate amount of time for each session to enable participants to complete their exercise program, warm up and cool down.
• Promote social interaction before and/or after sessions, when it is possible and appropriate to do so. For example, this might include making tea and coffee facilities available to participants after their sessions.
• Adhere to brand/trade mark guidelines, as provided by COTA Tasmania, when promoting Living Longer Living Stronger.

By submitting this Agreement, the Provider or their authorised representative confirms that they understand and will comply with their responsibilities as a Program Provider.

The Commencement Date will be the date that the Agreement is completed and submitted to COTA Tasmania (through this online form), provided that all conditions outlined in the Agreement and Terms and Conditions have been met and all required information has been provided.

The Agreement can be renewed on an annual basis, provided that:

• The Provider pays all applicable Provider fees.
• The Provider confirms and updates their information, as applicable, by completing the Provider Accreditation Agreement form each year.
• The Provider continues to comply with the Agreement and Terms and Conditions.

The Provider can terminate this Agreement by providing thirty (30) days notice to COTA Tasmania by email, phone or in-person.

COTA Tasmania can terminate this Agreement by providing thirty (30) days notice to the Provider by email, phone or in-person. All or part of any applicable Provider fees for the remainder of the current registration period (year) may be refunded to the Provider, dependent on the circumstances of the termination.

If the Agreement is terminated by either party, then the Provider must no longer use the Living Longer Living Stronger brand or associated materials.
